How to book

 
 

Location and opening hours

 

I work Tuesday 12-6.30pm,Wednesday and Friday 9.30am-6.30pm and Saturday 9.30am-5pm at Jenny Hampton Massage, 5 Post Office Court, St Johns Street, Whitchurch SY13 1QT.

 

Payment

 

Payment is made through my online booking system, or I take card payments, bank transfer or cash on the day.

 

Initial Consultation & Follow Ups

 

Arrive 10 minutes early for your first session to give time to carry out the initial consultation. I am not available to start the session more than 10 minutes early and the door to the building may be locked before then.

For follow up sessions, arrive 5 minutes early for a catch up consultation.

If at any point after you have booked the session you have COVID-19 symptoms, please cancel, as soon as possible. Also let me know if you test positive for Covid within 7 days of your treatment.

 

Cancellations

 

If you cancel or reschedule with 2-24 hours’ notice, I charge a 50% cancellation fee. Under 2 hours I charge the full session fee.

Once you have booked a session either by phone, text, email, messenger, in person or through the online booking system and have agreed to the terms and conditions, you are committed to attending that session. An email reminder is sent the day before as a courtesy but whether or not you receive it you are still responsible for attending or cancelling your appointment with at least 24 hours notice or you will be charged the cancellation fee.

If you are ill, please cancel your appointment, even on the day of the session, and you won’t be charged. Cancellation fee may be waived for other emergency circumstances, at the discretion of Jenny Hampton Massage.

 

Privacy and Consent

 

I take my professional obligations and the privacy of my clients very seriously. I will always discuss your treatment with you, ask you for feedback and make sure you are clear about what will happen during the session.

I will never share your information with anyone without asking you first. I keep your records in password protected software programs that only I can access. For more information, see my Privacy and Cookies Policy.

 

Client/Practitioner Agreement

 

You will be asked to agree to this when you book online, or I will ask you to read it at your initial consultation.

I understand that the massage practitioner is providing massage therapy services within their scope of practice. I therefore consent for my therapist to treat me with massage therapy for the purposes noted and agreed in the initial consultation.

I acknowledge that the practitioner is not a physician and does not diagnose illness, disease or any other physical or mental disorder and that massage is not a substitute for a medical examination.

I understand that no guarantee has been provided to me as to the results of the treatment and that with any treatment there can be risks.

I understand that the practitioner must be fully aware of my existing medical conditions and that I am responsible for keeping the massage practitioner updated on any medical changes.

I have read the above information, noted consent and have had the opportunity to clarify any concerns.

By signing this form, I confirm my consent to treatment and intend this consent to cover the treatment discussed with me today and such additional treatment as agreed with my therapist. I also confirm I understand and agree with the Privacy Policy set out in the Privacy Policy document. I understand that at any time I may withdraw my consent and treatment will be stopped.
